Request to Cancel Voter Registration :
Any registered voter may request that his or her name be removed from the voter registration rolls. The voter may complete a Request to Cancel Voter Registration form and send it to the County Election Board in the county where he or she is registered to vote, requesting that his or her voter registration be canceled. The request must be notarized.
Request to Cancel Registration of Deceased Voter :
** A deceased voter’s registration may be canceled upon request of the voter’s next of kin.
** The deceased voter’s next of kin may complete the Request to Cancel Registration of Deceased Voter and send it to the County Election Board in the county where the deceased voter is registered.
** The request must be notarized or witnessed by two persons.
** Use of the official form is not required, however.
** The deceased voter’s next of kin may send a written statement regarding the voter’s death to the County Election Board in the county where the deceased voter is registered.
** The statement must include the voter’s complete name, address and date of birth.
** This statement must be notarized or witnessed by two persons.
** A certified copy of a death certificate may also be provided to the County Election Board.
** The next of kin also may complete the “Request to Cancel Registration of Deceased Voter” at his or her polling place on election day.
Voter Assistance in Oklahoma :
** Oklahoma voters who have visual or physical disabilities may vote independently and privately at their polling places on election day and during early voting at the County Election Board office preceding every election.
** Oklahoma’s new Hart InterCivic eScan A/T voting device is equipped with an audio tactile interface (ATI) that enables a voter to listen to instructions for using the ATI controllers and an audio version of the ballot, to make selections for each race or question on the ballot, to review all selections and make changes if necessary, and finally to cast the ballot privately and independently.
** The voter’s ballot selections are recorded electronically in the device’s memory and included in the results for the precinct.
** No record will exist to tie an individual voter to a specific ATI ballot.
** Voters who use “sip and puff” or tactile input switches may plug their own assistive devices into the ATI controller and use them to operate it.
** While a voter is using the ATI device, other voters may continue voting and may insert their paper ballots into the eScan A/T at any time.
** Some voters choose to vote with the assistance of another person.
** The Precinct Officials at your polling place are trained to offer assistance to voters who request it.
Assistance is available at all elections for :
** voters who have physical disabilities and are unable to mark their own ballots
** voters who have visual impairment
** voters who have physical disabilities or infirmities and are unable to enter the polling place
** voters who cannot read
Choosing Your Assistant :
If you need assistance to vote, you may choose anyone you want to help you, as long as the person is not your employer or an agent of your employer or an agent of your union. You may be assisted by a relative, a friend, or by one of the Precinct Officials at your polling place.
Polling Place Accessibility :
** Oklahoma election officials are working to locate all polling places in buildings that are accessible to voters with disabilities.
** Some voters with physical disabilities still may be unable to enter their polling places, however.
** These voters may be assisted by two Precinct Officials outside the polling place.
** This procedure is called “curbside voting.”
** Call your local County Election Board for more information.
Absentee Voting :
** Absentee voting is also a choice for voters with visual or physical disabilities.
** Any registered voter may request absentee ballots.
** You don’t have to give a reason for voting by absentee ballot.
** However, voters who have physical disabilities may give that as a reason for voting by absentee ballot.
** By stating this reason, you may take advantage of some special conditions that may make absentee voting even easier.
Other Assistance for Voters ;
** If you need help to fill out the Oklahoma Voter Registration Application form, you may ask a friend or relative to help.
** The helper must write his or her name and address in the space provided on the application form.
** If you want to apply for voter registration but are unable to sign the form or even to make a mark on the form, contact your local County Election Board or the State Election Board for more information.
** The State Election Board has installed a telephone line dedicated for use with a TDD. The TDD number is (405) 521-3028.
** Voters who are deaf or hearing impaired are encouraged to call the State Election Board for information and assistance.
** Facts about Proof of Identity for Voting in Oklahoma
** Oklahoma’s proof of identity law (sometimes called the “voter ID” law) was contained in State Question 746 (Legislative Referendum No. 347).
** The question appeared on the ballot at the November 2, 2010, General Election, and it was approved by 74% of the votes cast.
** The proof of identity law requires every voter who votes in person at the precinct polling place or during early voting at the County Election Board to show proof of identity before receiving a ballot.
** There are three ways to prove your identity.
PHOTO IDENTIFICATION :
A document used for proof of identity for voting must have been issued by the United States government, the State of Oklahoma, or a federally recognized tribal government.
The law requires a document used for proof of identity for voting to contain the following information :
** The name of the person to whom it was issued
** A photograph of the person to whom it was issued
** An expiration date that is after the date of the election, unless the identification is valid indefinitely
The law requires the voter’s name on the proof of identity document to “substantially conform” to the voter’s name in the Precinct Registry. In other words, your name on your proof of identity must match your name in the Precinct Registry.
COUNTY ELECTION BOARD VOTER IDENTIFICATION CARD ;
In addition, voters may use the free voter identification card they received by mail from the County Election Board when they registered to vote. The law allows use of the voter identification card even though it does not include a photograph.
AFFIDAVIT WITH PROVISIONAL BALLOT :
** If you do not have or if you refuse to show proof of identity, you may vote by provisional ballot and prove your identity by signing a sworn affidavit.
** Provisional ballots are sealed inside special envelopes and are not put through the voting device.
** After election day, County Election Board officials will investigate the information provided by the voter on the affidavit and either will approve the provisional ballot for counting or will reject it based on the outcome of that investigation.
A provisional ballot cast by a voter who declines or is unable to produce proof of identity shall only be considered verified and approved for counting if :
** The voter’s name on the affidavit substantially conforms to the voter’s name in the voter registration database.
** The voter’s residence address on the affidavit substantially conforms to the voter’s residence address in the voter registration database.
** The voter’s date of birth matches the information in the voter registration database.
** The voter’s Oklahoma driver license number or the last four digits of the voter’s Social Security number on the affidavit matches the information in the voter registration database. (This provision does not apply if the voter was not required to provide a driver license number or the last four digits of the voter’s Social Security number at the time of registration.)
** The provisional ballot meets the eligibility requirements set forth in Section 7-116.1 of Title 26 of the Oklahoma Statues.
